RHF Request for Funding

Reading Hospital employees seeking funding less $10,000 from Reading Hospital Foundation should complete this form.  The Foundation supports projects which impact the patients, families, and staff in the Reading Hospital. Due to the increased requests for funding, please do not request funds for salaries, or capital improvements such as renovations. Please work through the purchasing department to obtain quotes prior to requesting funds.
